HOW SUPPORTED LIVING WORKS
Supported living gives adults with developmental disabilities the opportunity to live in a home of their own while receiving the individualized support they need to be safe, successful, and as independent as possible.
For many families, the process can seem complicated at first. TLC helps families understand each step, work with the Regional Center, develop the right supports, and move toward a living arrangement that fits the individual’s needs, choices, and goals.
Below is a simple overview of how the process works from beginning to end.
The Process: Step by Step
1. Talk With Your Regional Center Service Coordinator
Contact your SDRC Service Coordinator and request a meeting to discuss adding Supported Living Services to the individual’s IPP.
2. Attend Orientation
The individual and family attend an orientation to learn more about Supported Living Services and what the process involves.
3. Choose an SLS Agency
You will have the opportunity to meet with and interview local Supported Living Service agencies and decide which one feels like the best fit.
4. Assessment
The SLS agency you choose will begin the assessment process to evaluate support needs and make a recommendation to SDRC based on their findings.
5. Approval
SDRC provides approval of Supported Living Services based on the recommendations provided.
6. Plan
The SLS agency joins the planning team and develops a person-centered Individual Service Plan.
7. Match
Your agency will begin the hiring process to match you with the right Direct Support Professionals.
8. Explore
You will begin looking for rentals within your community that meet your budget and accessibility needs.
9. Move
Once a home has been secured, the move begins! Your agency will help with the process, along with setting up utilities.
